How to file a TD Protection Plans Claim

If you are an estate representative making a Life claim or you are making a Critical Illness, Disability or Accidental Dismemberment claim, follow these steps to file your claim.

1. Download and print the claim form:
Written proof of your claim must be submitted to TD Life Insurance Company. You can download and print your claim form or call us at 1-888-983-7070 to have it mailed to you. We encourage you to review your Certificate of Insurance and ensure your claim is submitted within the required timeframe.

2. Complete the form:
Please follow the instructions on the claim form and ensure it is completed in full. If the form is not completed in full, this could result in delays processing your claim.

3. Submit the form:
Return the completed claim form with required supporting documentation to the following address:

P O Box 1
TD Centre
Toronto, ON M5K 1A2

4. You may be contacted:
Once received, TD Life Insurance Company will review your claim. We may contact you for additional information that may be required to assess your claim.

5. Receive a written decision:
We will contact you in writing to advise you of our decision relating to the claim.
